Social Work Advocates Efforts Stave off Social Work Job Loss

A recent Op-Ed in the NY Daily News, “When social work is a felony” and news accounts by NY1 paint a fearful and distorted picture of an issue NASW-NYS and the NYS Society for Clinical Social Work have dedicated a considerable amount of energy and resources, in conjunction with the NYS Education Department- Office of the Professions, allied social work organizations, and executives of agencies impacted by the corporate practice statute to resolve the issue. The resulting waiver process, due in large part to our legislative work, was agreed upon by all participants in this Social Work Alliance as an acceptable solution to this unintended consequence of implementing the social work statute (and that of other mental health professionals). To date nearly 1,000 agencies have applied for a waiver, including that of the Op-Ed’s author. The primary objection voiced by many agencies to the 4 page, no cost waiver application process has been related to the required submittal of a moral character attestation by directors, trustees and officers. It is important to note that the waiver process is far less stringent than what is required by any other licensing or regulatory body.

To provide a brief history of the issue, as of September 1, 2004, the practice of licensed master social work and licensed clinical social work became restricted to licensed persons (or those exempt from licensure under the law); however, NYS “corporate practice” law specifically prohibits the practice of licensed professions by a business or corporation.  This is problematic because many community based providers who have a long history of providing social work and mental health services, are (in many instances, unknowingly) in violation of corporate practice statute.  As a consequence of the agencies inadvertent violation of law, the State Education Department could not accept clinical experience obtained in, or issue permits to practice in such settings, impacting the ability of new practitioners to enter the workforce and preventing others from attaining their clinical license.

With such a significant amount of the workforce unknowingly providing services illegally, taking punitive actions such as the consequent alternative of layoffs would have created a huge disruption in the availability of services across the state and would not have been in the best interest of the individuals, families and communities being served nor that of affected providers. Therefore NASW-NYS worked in conjunction with the Social Work Licensure Alliance (social work and agency provider groups as well as other relevant stakeholders) and the NYS Education Department to address the situation. 

The possibility of seeking an exemption to the corporate practice law like that which a few select professions have been granted was explored. However it was clear that there would be opposition from certain stakeholder groups that would make the fight for such legislation lengthy and given the enormity of the potential impact of this situation on consumers and social workers, a more immediate remedy was needed.  To that end, a bill was drafted at the end of the regular 2009 session (S. 5921 and A.8897).  The bill proposed amending Title VIII of the Education Law to allow the State Education Department to register entities that do not possess operating certificates or authorization to employ licensed individuals.  However this was an unacceptable resolution to the legislature. It was in June of 2010 that the NYS Legislature passed the law addressing the practice of clinical social work and other scope protected tasks including the provision of mental health counseling and psychotherapy in currently unauthorized corporate settings. The law also allowed the Education Department to issue a waiver from corporate practice prohibitions to qualified not-for-profit and education corporations.

The Board of Regents approved permanent regulations to implement the law, and as such, the Department began accepting waiver applications beginning February 2, 2011 with an initial deadline of June 16, 2011 which did not in our opinion provide a sufficient timeframe. NASW-NYS went back to the legislature and successfully worked on an extension bill that pushed the deadline to February 1, 2012.  

NASW-NYS in conjunction with the NYS Association of Deans of Schools of Social Work produced an informational video regarding this issue to clarify who the law pertains to, which corporate entities might require a waiver and the application process. We collectively engaged in extensive outreach to field agencies of the schools of social work, members of the Social Work Alliance, members the NY Council of Nonprofits (who hosted the video on their YouTube channel) as well as our respective members to promote awareness of the waiver and prompt affected agencies to complete the brief (4 page no-cost application) prior to the February 1st deadline.

This workshop will discuss countertransference, defining it with specific clients in mind and using art to create the environment necessary for the relationship to thrive.  It will involve a series of exercises designed to help us use our feelings in relation to our clients in order to assist them.

Please bring your favorite magic markers or other instruments of color.

Educational Objective:  Participants will learn an innovative method to transform these emotions in order to be present and create the distance necessary for integration and insight to occur.  They will also use a “process over product” orientation with simple materials, such as markers and paper.  This process of allowing the marker to have a mind of its own brings us closer to understanding our own symbols and, therefore, the client’s.

Sarah Zahnstecher, LCAT is a Licensed Creative Arts Therapist with over 20 years experience.  She has Master’s degrees in Art therapy as well as Fine Arts.  She now works in private practice in Brooklyn, NY after decades of agency and hospital work with a wide variety of populations.  She has presented numerous workshops on diverse subjects to train and develop staff and professional associates.  She has supervised both Art Therapists and Social Workers.

During our discussion, we will use the couple in the film “Blue Valentine” as our example of a couple in distress and how to increase our assessment and intervention skills from Object Relations Theory and Attachment Theory perspectives.  A brief understanding of gender role, class and economic stressors will also be incorporated into this presentation.  We will discuss how our expectations and choice of a love object are formed by our primary relationships, how individual members in a couple can become emotionally dysregulated, and how we can help them to improve communication skills and relationship behavior.

Hope Eliasof, LCSW, LMFT, has been a relationship therapist, parent educator and instructor for over 25 years.  She received her Master’s degree in Social Work at the Wurzweiler School of Social work and her Bachelor’s degree from CUNY at Brooklyn.  She also received postgraduate certification in psychoanalytic Psychotherapy from The Institute of Mental Health Education in Englewood, NJ and a Supervisor Certification from the National Institute for the Psychotherapies in New York City.  Ms. Eliasof has been an Adjunct Professor at New York University’s Silver School of Social Work’s Masters and Post-Masters Programs and she is a Part-Time Supervisor at Pascack Mental Health Clinic in Park Ridge, NJ.  She is Former Director of the Parenting Center, Temple Beth El in Closter, NJ and Parenting Group Leader at the Jewish Community Center on the Palisades.  She continues to present Parenting and Relationship Workshops and maintains a private practice in psychotherapy for individuals, children, families and couples in Bergen County, NJ.  Her work has appeared in The Parent Paper and About Our Children.

The emotional tasks of making and maintaining our most important adult relationships are multiple, unremitting, daunting and profound.  Our need for the ‘secure base’ continues to be pressing throughout our lives.  Yet the risks we run--in choosing our significant others, in exposing ourselves to intimacy, in trying to offer and obtain love—are substantial.  How we manage loss, rupture and disappointment in our adult attachments is a major theme of therapy and a major challenge of adult life.  This presentation will address some of the issues of adult attachment through clinical and fictional case material.

BIO

Hilary Ryglewicz, LCSW is a psychotherapist with over 30 years clinical experience, whose background includes both psychodynamic and cognitive approaches.  Ms. Ryglewicz is a supervisor and faculty member of the Couples Program of the Training Institute for Mental Health in New York City.  She maintains a private practice with individuals and couples in Manhattan and in Nyack.

This workshop will discuss: the dynamics of creativity in psychoanalytic treatment and how the model of artistic creativity may be useful in understanding the analytic process. Several phases of the creative process are planned for discussion; first as it is manifest in art and then as we see it in the analytic process. A case example will be presented with several recommendations for enhancing analytic creativity.


Educational Objective: Participants will learn an innovative model of creativity in analytic practice. They will also hear clinical vignettes illustrating the model, and acquire special skills that will enhance their own creative potential.

George Hagman, LCSW is a psychoanalytic clinical social worker in private and public practice in New York City and Connecticut. He is also the Director of Out-Patient Services at the Greater Bridgeport Community Mental Health Center in Connecticut. He is a member of the faculty of the Training Institute of the National Psychological Association for Psychoanalysis as well as the Training and Research Institute for Self Psychology and he is Scientific Program Chair of the Connecticut Society for Psychoanalytic Psychology. George has published numerous articles in a number of major psychoanalytic journals such as the International Journal of Psychoanalysis He has presented at meetings of the International Psychoanalytic Association, American Psychoanalytic Association, the Annual Meetings on the Psychology of the Self and other national professional meetings.  In March 2012 he will present at the annual conference of the International Association of Relational Psychoanalysis. His book Aesthetic Experience: Beauty, Creativity and the Search for the Ideal was published in 2005 by Rodopi Press as Volume 5 of the series Contemporary Psychoanalytic Studies, and he is the co-editor of a special issue of Psychoanalytic Inquiry titled “Creativity, Art and Psychoanalysis: Current Perspectives” published by the Analytic Press in 2006. In June 2010 Routledge published, The Artist’s Mind: Creativity, Modern Art and Modern Artists.

Medicare Webinar 
February 28

On Monday, February 28, from 9:00 to 11:00 a.m., National Government Services will provide a seminar and teleconference on Medicare.   

Kathy Dunphy and James Bavoso of NGS will provide an overview of provider enrollment, PECOS, documentation, and other provider responsibilities, and will discuss the future direction of Medicare, including electronic billing.  Following the initial presentation there will be a question and answer period.  

This seminar will take place at 1 Liberty Plaza in Manhattan, 14th floor, The Presentation Room.  Nearest subway stop is Fulton/Nassau on the 4, 5, or 6 train.   Photo ID required to enter the building.   

Members unable to attend in person will be able to participate by teleconference while following a Power Point presentation available on the NYSSCSW website.

Ritz Gazarik, LCSW, will facilitate the discussion. She is the director of Family Life Cycle Mental Health Associates, a multicultural practice of family therapists. A graduate of the Ackerman Institute for the Family and the Family Institute of Westchester, she also teaches at the postgraduate program of the Hunter College School of Social Work and at the social work school at Charles University, in Prague. The Topic: "I Love Him, I Love Him Not - Intimacy Issues of Single Mid-Life Women"
New choices and opportunities abound in today's world for mid-life women. Yet often women experience difficulty meeting available men, feel ambivalent about their current relationships or are unsure about being in a relationship at all. Women struggle with fantasies and inhibitions related to life in former times, while living in a culture that continues to idealize youth, beauty, style, and money. These themes will be elaborated on, with reference to self-psychology, contemporary oedipal theory and object relations in the treatment of two or three women in their forties and fifties. The question becomes what do these women want now?

Topic: The Role of Forgiveness in the Working Through of Traumatic Events – A Dialogue

Presenter: Gwenn A. Nusbaum, LCSW, BCD

Through intensive therapeutic work, opportunities to transcend traumatic events and achieve post-traumatic mastery are possible for people.  This workshop will discuss components of post-traumatic growth as conceptualized by Richard Tedeschi, and link the question: Is forgiveness necessary to the process of working through and healing from traumatic events – especially those involving victimizations – and if so, why?  A brief statement about “forgiveness therapies”, and the controversy surrounding it, will be offered.  Clinical vignettes will illustrate instances in which traumatized patients have grappled with the question of forgiveness and what it has meant to both forgive and not to forgive. Significant time will be given to participants, for a dialogue about these inquiries.

Gwenn A. Nusbaum, LCSW, BCD, CGP, has treated survivors in individual, couple and group therapy for over 25 years.  She has published on the subject of the efficacy of psychodynamic group treatment for women with histories of childhood sexual traumas, the relationship between sexual abuse and eating disorders and the need for defiance in women with abuse histories.  A relational psychotherapist and psychoanalyst, Gwenn maintains a private practice in the East Village and is senior faculty of the Postgraduate Group Therapy Department as well as Training Analyst at the Psychoanalytic Psychotherapy Study Center.

The Psychiatric Medicine Dilemma: Children and Adolescents

Our Presenter will be Dr. Kevin T. Kalikow, Clinical Assistant Professor in Child Psychiatry at New York Medical College, who also teaches at Columbia University College of Physicians and Surgeons.  He is the author of:  Your Child in the Balance: An Insider's Guide for Parents to the Psychiatric Medicine Dilemma, and Kids on Meds: Up-to-Date Information About the Most Commonly Prescribed Psychiatric Medications. 

Dr. Kalikow writes that both books were written to address the thorny topic of prescribing psychiatric medicines to children.  The former volume helps parents and clinicians understand how doctors decide to prescribe medicine and whether medicine is the right treatment for their child.  The second reviews the use, side effects and theory behind the many medicines used in child psychiatry.

Borderline Personality Disorder: Two Treatment Models

Dr. Marschke will present a brief overview of current research and its relevance for understanding and treating this patient population.  The two models will be described and contrasted.  Discussed will be a rationale for matching different models of treatment with differing patients who share a diagnosis of Borderline Personality Disorder.

She is an MSW graduate of Fordham University and completed her Ph.D. at Smith College School of Social Work.  While in that program she conducted a follow-up study of formerly hospitalized adolescents diagnosed with BPD, who were treated using James Masterson’s psychoanalytic treatment model.  Since then she has introduced the DBT model to mental health providers and conducted groups with patients with BPD and their parents.  Currently she is in private practice in Kingston, New York, and works for the EAP Services of the Mid-Hudson Valley.  She has been a past president of the NYSSCSW and of its Mid-Hudson Chapter.

Gloria Robbins is a longtime private practitioner in the Hudson Valley, trained in Psychodrama, Bio-energetics, Rhythmic Integration, Jungian Analysis and Play Therapy.  Ms. Robbins has studied with many Spiritual Masters, and has conducted workshops and trainings internationally. She has led The Women’s Group in Poughkeepsie for over thirty-five years. 

She will present her teaching video “Ego In Motion”, showing four pre-school children in various stages of ego development.  The viewer observes the presenting problem, interventions by the therapist and changes that occur over time.   The less developed ego demands a more passive presence by the therapist, in order to enhance and strengthen the ego.

The more developed ego demands more interaction between the therapist and child in the working through of unresolved conflicts.  The principles of play therapy will be discussed during the viewing and can be generalized to treatment with children of all ages, adolescents and adults.

     Alan Spivack, LCSW, a founder of the Family Therapy Center of Poughkeepsie, presents his workshop on Theraplay, a fun, innovative short term treatment method that uses structured play therapy methods with its participants.  After an extensive pre-therapy evaluation, there are 10 sessions which involve therapist and child, parent and child, and sometimes parents and therapist.  
           This method is very helpful for use with adopted/foster care children with difficult symptom pictures, autistic children, children with regulatory disorders and sensory integration problems, adolescents or children with oppositional and disassociated problems and children with emotional or physical trauma histories.  The method involves observed, nurturing touch and considers it essential for success.  
 
All mental health clinicians and students are welcome.

Exploring the Neuroscience of Psychotherapy:Why What We Do Works and Doesn’t Work

     What do premature infants, colicky babies, children with ADHD, people with autism and teenagers have in common?  What does the music of the 50’s, early 60’s and today’s rap tell us about pair bonding, predatory behavior and the use of our voice during sessions?  What can a 1200 pound polar bear and a wolf teach us about the deepest aspects of our work?  These seemingly disparate questions are answered as we discuss the fascinating work of Dr. Stephen Porges, a neuroscientist and pioneer in the study of the psycho-neurobiology of social engagement and therapeutic change.  Illustrating Porges’s “Social Engagement System,” we will discuss a view of the mind-body ‘resiliency zone’ that is radically changing our understanding of what happens in clinical treatment.

     Terry Nathanson, LCSW, has been an active student and practitioner of mind-body models of treatment for over 25 years.  He integrates Gestalt, Internal Family Systems, trauma based somatic approaches, interpersonal neurobiology and eastern contemplative perspectives.  Fascinated with the limitations of talk therapies and the hyperarousal of cathartic therapies, Terry has been passionately drawn to the inquiry of why psychotherapy works and doesn’t work.  He has brought his neurobiological orientation to his work with populations as diverse as reactive couples, disordered eaters, adult and juvenile sexual offenders and victims of attachment trauma and maintains a private practice in Ossining and Port Jefferson, NY. Exploring the Functions of Out of Control Sexual Behavior

     Individuals presenting with driven, out of control sexual behavior may put their work, relationships and even their lives at risk. Working with these patients involves attending to the possible functions of these behaviors as they relate to patients' struggles with intimacy, attachment and sense of self. Clinical material will be presented that represents these issues and provides an opportunity to explore both the functions of the symptoms as well as treatment approaches.  Additionally, ecological factors influencing these symptoms including advancements in technology and the use of the internet will be explored.

Michael M. Crocker, MA, LCSW is a private practitioner working with individuals, couples, groups and organizations. He provides psychotherapy services that address trauma, cross addiction, out of control sexual behavior, as well as sexual inhibition and intimacy anxiety. He also functions as an Adjunct Professor for the Social Work Department at Iona College teaching courses on Trauma, Violence and Resilience. He is currently a doctoral candidate at The University of Pennsylvania where he is studying the connection between out of control sexual behavior and attachment disorders.

Susan Malewicz,  LCSW, CASAC, CSAT, SAP is a licensed clinical social worker, a certified alcohol and substance abuse counselor, a certified gambling counselor, a certified sex addiction therapist and a substance abuse professional.
Susan received her training with Patrick Carnes PhD at  the International Institute for Trauma and Addiction Professionals in 2001 where she remains a member .  Susan went on to receive her master's degree in social work from the University of Stony Brook  where she has returned to lecture on the subject of sexual addiction and treatment. She is  a member of the Society for Advancement for Sexual Health, the National Association for Social Workers  and serves as a committee member for the Criminal Justice Addiction Work Group.
Susan has developed, coordinated, and conducted several outpatient programs for level I, II, and III sex offenders at Passages Counseling Center, Crossings Recovery Centers  and for The Center for Addiction, Recovery and Empowerment

     The workshop will cover the dynamics and effects of separation and divorce on the partners, children and especially the therapist. Divorce is about loss, grief, love and hate, and the problem of moving on when letting go is so difficult. We will apply object relations concepts to practice with couples, families and children in the throes of love gone bad. Topics to be covered include: marital love and hate; the therapist's beliefs and working hypothesis in divorce situations; expected and unexpected break ups in couple therapy and the strains on therapist's holding and containment; working with children and parents before and during a divorce situation.

     We will use the presenter's and participant's cases in the clinical discussion and application of workshop ideas. Participants will have an opportunity to add related practice topics during the discussion.

"Web Site Development to Promote Your Private Practice"

A website is an important promotional tool. More and more potential clients are locating therapists on the Internet. But many clinicians hesitate to develop a website, not knowing what to do and how to get started. Others who already have a website don't know how to draw traffic to their website or how to use their website to draw new clients into the office. In fact, building a basic, simple and effective promotional website isn't difficult, and can be done with a moderate investment of time and money and minimal technical expertise.

In this meeting, clinical social worker and veteran web developer Michael Greene will provide guidance about how to build and maintain a website, how to draw potential clients to your website, and how to integrate your website into your online and "off-line" marketing strategies, as well as fielding questions from the audience.

Prior to becoming a licensed clinical social worker Michael Greene's first career of 20 years in the technology industry included working on Internet projects in the corporate environment and for several non profit organizations that he co-founded. In the past five years Michael has worked as a social worker serving families of children with Aspergers and Autism as part of a multi-purpose private group in NJ, and at a community based clinic in Queens. Crayton Rowe, M.S.W., B.C.D-P, will present his contribution of the undifferentiated selfobject which is a fundamental selfobject that is integral to understanding addictive, suicidal, compulsive and attention-deficit/hyperactivity (ADHD) behavior. He will present a brief film that demonstrates its development in infants. A discussion of the treatment value of this fundamental selfobject will follow the film, and participants will be able to present clinical examples.

Crayton Rowe is founder of the American Association for Psychoanalysis in Clinical Social Work; founding member of The New York Institute for Psychoanalytic Self Psychology; and member the International Psychoanalytical Association. He is the author of Treating the Basic Self and co-author of Empathic Attunement, The "Technique" of Psychoanalytic Self Psychology. He is a contributor to Progress in Self Psychology and author of a number of psychoanalytic papers.

"A Psychotherapist's Experiences of Loss:  Midlife and Beyond"

Sheila's presentation will be about life and connection, separation and death and the moments in-between. Loss will be addressed from the vantage point of midlife: a time during which ones' ability  to deny death is profoundly challenged.  after the presentation, there will be time for discussion, as prior presentations on this have stimulated interesting thoughts, feelings and responses. 

Sheila felberbaum, first educated as a nurse, maintains a general psychotherapy practice in smithtown, ny, with a specialty in bereavement and the treatment of patients with life challenging medical diagnoses.  this should be not only interesting, but informative, educational, and provocative.  This is a NOT- to-be-missed opportunity!

TITLE: “Eating Disorders as Metaphor: Treatment That Has Little to Do With Food”

ABSTRACT
The prevalence and persistence of eating disorders in young people demands that we take another look at the effectiveness of treatment at every level of care. This presentation aims to show that eating disorders are addictive in nature, but not by the usual criteria for addiction commonly ascribed to other addictions. Effective treatment includes alternative perspectives and methods such as cognitive narratives, art therapy, appropriate family education and integration, and peer support. Ultimately, optimal treatment works best within the paradigm of eating disorders as metaphors. To be effective, clinicians must acknowledge the important stories these disorders tell.

Jennie Kramer, MSW, LCSW is a psychotherapist specializing in the treatment of Eating Disorders. She is Founder and Director of Metro Behavioral Health Associates in Scarsdale and NYC, which offers individual, family and group therapy, nutritional counseling, medication management and targeted Intensive Outpatient Programs specifically for the treatment of eating disorders.

TITLE: “Mistake or Opportunity: Choice Points in Psychotherapy”

DESCRIPTION: Whichever approach or theoretical perspective we clinicians may hold, it is in the nature of the psychotherapy process to make what we consider to be “mistakes.” This may negatively affect our image of ourselves and prevent us from talking openly about these mistakes with colleagues and patients. In this presentation I will use several events that occurred over the course of a long-term therapeutic relationship as a catalyst for attendees to share instances of mistakes in their own practices, and to examine if/how they became therapeutic opportunities. Together, we will discuss what constitutes a mistake, what constitutes an opportunity, and how we can define what it is to be a “good enough” therapist.

BIO: Rhona Engels, LCSW, ACSW has been in private practice in NYC for 27 years, working with individuals, couples and groups as well as providing individual and group supervision. She has taught in the graduate School of Social Work at New York University, at the Fifth Avenue Institute and at the Karen Horney Institute. She has published frequently in clinical journals and led many workshops, all providing her with extensive experience in making mistakes and trying to transform them into therapeutic opportunities.

TITLE: “Avoiding Diagnostic Deficit Disorder: The Accurate Assessment and Treatment of Sub-Syndromal Mood Disorders”
     Often patients in therapy do not benefit maximally, and continue to not function to their best potential, due to a co-morbid, unrecognized, misdiagnosed or mistreated biochemical imbalance.  Clinicians misdiagnose adults with full spectrum bipolar disorder nearly 70% of the time.  Usually they incorrectly diagnose and treat the patient for “depression.”  The misdiagnosis rates are higher in children, seniors and people with organic brain disease.  This high rate of misdiagnosis also does not include the rate of mis-medication, which is higher yet.  In this presentation we will review the symptoms (often obvious and occasionally subtle) that promote making a correct diagnosis and the medications that might be best used for treatment. We will also review how to distinguish between palliation of symptoms versus treatment of a disorder to remission.
     Dr. Steve Weisblatt is currently Clinical Assistant Professor of Psychiatry at Downstate Medical Center.  In addition to being the author of the former "Clinician's Corner" column in Bipolar Magazine, he has spoken widely in the US, Canada and Europe on the subject of accurate diagnosis and effective treatments for patients with mood disorders.  Currently, Steve is in private practice with offices in New Rochelle, NY and Pocono Lake, PA.
